This letter contains information for parents and guardians regarding picking up their student's medication at the end of the school year as well as information in preparation for the next school year.
Parents, guardians, and healthcare providers should use this form when their student requires medication to be administered during the school day, during a school sponsored event, or class trip.
These documents are guidelines and a list of information school health personnel and your healthcare provider can use in determining if a student is self-directed and may carry his or her required medication in the school setting.
This form should be printed and filled out by the parent/guardian who wishes to designate another adult to administer medication to his or her child while on a school sponsored event.
This form requests information and documentation from your child's health care provider regarding emergency medication, emergency care plan and/or asthma action plan for the school year, when needed. The form may be printed and taken to the health care provider for him or her to fill out. It can then submitted to the health office at your child's school for review by the school nurse. Your child's school nurse is available to answer any questions you may have in regards to emergency care of your child.
